Your facility is a large children’s hospital in north Texas. Until recently your facility was part of a larger health care facility run by the state. The facility is now corporately owned and operated. During this reorganization period a major fire occurred at a local elementary school. Over 100 injured children were sent to your facility for care. You encountered many problems: care was delayed and inappropriate for many children because of the nature of their injuries due to lack of parental consent for treatment and smoke inhalation and burns. Your task is to develop a disaster plan specifically to address the deficiencies and to reshape your public image following the fire at the elementary school.

Imagine you and ten of your family members are in the kitchen cooking a massive feast for thirty people. It is pure chaos in the kitchen because nobody is taking charge to ensure the needed dishes get done in a timely manner.
Now switch gears and imagine you are on a multidisciplinary team at your local clinic focusing on increasing local youth access to fresh fruits and vegetables. This group does not have a leader.
In your initial post, discuss whether or not it is possible to accomplish what is needed without a leader? Why or why not?

Case Scenario:
The patient is a 72-year-old widowed Native American woman named Rose Smith. Mrs.Smith lives alone near Traverse City. Her son, daughter-in-law, daughter and their children all live outside of Michigan.
Her income is $15,000 per year and her healthcare coverage is traditional Medicare (Parts A, B and D). She does not have access to a car. Mrs. Smith is diabetic and routinely receives care at the local health clinic. For hospital care she must travel to Traverse City.
Mrs. Smith tripped in her home and broke her left hip. A surgical repair was necessary and was performed at Munson Medical Center for surgery with a 7-day inpatient stay. After Mrs. Smith’s inpatient stay, she was discharged to Munson Health and Rehabilitation for short-term rehabilitation stay of 14 days. Upon discharge from Munson, in-home physical and occupational therapy was scheduled for a period of 2 months. The discharge plan for Mrs. Smith anticipates that she will be able to be discharged from home care post evaluation. After that date, it is anticipated that she will live independently and care for herself at home.

You completed your full head-to-toe assessment skills demonstration last week and now will document your results. Continue to document only the objective findings for this section without bias or explanation. Remember if you can’t feel something then it is “nonpalpable,” if you can’t hear something just state they were not heard such as no bowel sounds heard (unless you listened for the full five minutes which we wouldn’t want to do for our purposes – then you could document absent bowel sounds). Be descriptive if necessary but at the same time be brief.
