CASE STUDY: Mary Ann a premigravida is in her firth month of her pregnancy, which has progressed without conplications. During her prenatal visit today, she tells you that she has some questions about her activity, work and hygiene. Indicate your nursing diagnosis, assessment, planning goals and implementation and evaluation for this patient.
2) Mary Ann is now in her eighth month of her pregnancy, which has progressed without complications. She states that she looks forward to the birth of her baby, and ask how to prepare for the labor and the delivery. 5th month of pregnancy:
There will be lot of rapid physical changes happens as fetus grow in size..
Assessment:
-Sciatic nerve will start feel pressure due to increase in baby size..weight will compress against the nerve,causing discomfort..
- feeling hot even during winter due to increased blood supply inside the body.
- the veins looks visible.varicose vein can be seen due to overweight.
- hair looks thick and there will be less hair loss.
- there will be Brittle nails
- you may feel mood swings but it is normal..
- weight will be doubled in the last two weeks of the 5 the months.
Goals and implementation:
- comfortable clothing like soft fabric and light coloured will keep your mood pleasant.
- fiber rich diet may prevent constipation
- good posture with back support will make you relaxed and feel comfortable.
- health and fitness is important so continue doing stretching exercises
- read some good moral books and listen to music can provide positive mind
- sleep to leftside everyday it improves healthy circulation to your uterus..keep pillows in your back and inbetween the legs..
- you may feel more hungry so eat healthy foods like fruits, vegetables and juice.. avoid junk food and oily food
- avoid coffeine, alcohol and smoking it is dangerous to baby
- maintain hygeine when you go to toilet..wash thoroughly with antiseptic soap and water to avoid infection..wear cotton innerwear to avoid bacterial growth.
Continue to stay happy, relaxed and stress free
8 the month is very important period due to plenty of changes will happen in the last trimester of pregnancy.
Assessment:
- feeling confused, annoyed.
- anxiety due to having a baby and nervous about delivery
- you feel urine leakage when you sneeze,cough.. understand that this is natural..
- fluid retention make your hands and feet swell
- frequent breathlessness due to fetus compress the lungs and diaphragm.
- heartburn and constipation is common
- false contraction due to uterus harden and release.
- colostrum continue to leak from nipple
- there will be less amniotic fluid leakage.
Goals and implementation:
- do breathing exercise to feel relax
- kegel exercises to relieve you from the incontinence.
- drink lot of water,fruit juices,stay hydrated always.
-eat short, fulfilling meals at regular intervals instead of large meal.
- do basic yoga exercise and stretching exercises
--apply lotions immediately after both to soften your skin
- take warm water both and avoid tub bath.
- good hygiene will prevent vaginal infection.avoid douche during pregnancy
- visit dentist to check for any tooth decay and get treatment done.
- have mild walking to promote normal vaginal delivery and you may feel relaxed.
