Can ACE (Angiotensin-converting enzyme) inhibitiors and angiotensin receptor blockers be used in hypertensive patients with renal insufficiency?
ACE INHIBITORS CAN BE USED IN HYPERTENSIVE PATIENT WITH RENAL INSUFFICIENCY
provide renoprotective effects (nonblood pressure-dependent ) with antihypertensive therapy in patients with more advanced renal disease.. but Angiotensin-converting enzyme inhibitors (ACEIs) are contraindicated in patients with bilateral renal artery stenosis due to the risk of azotemia resulting from preferential efferent arteriolar vasodilation in the renal glomerulus due to inhibition of angiotensin II.
chronic renal failure can be prevent by ACE inhibitor
INHIBITION OF RENIN ANGIOTENSIN SYSTEM
1. sympathetic blocker decrease renin release
2. renin inhibitory peptides and renin specific antibodies block renin action-interfere with generation of AU from angiotensinogen
3. angiotensin converting enzymeprevent generation of active principle AII
4.AT 1 antagonisy block action of AII on target cell
5. Aldosterone antagonist block mineralocorticoid receptors
