an advance health care directive ia also called as living will ,personal directive,advance directive ,medical directive,advance decision ,is a legal document in which a person specify what actions should be take for their health if they are no longer able to to make decision for them selfs because of their illness or incapacitting .

- in the U.S it hs a legal status itself ,whereas in some countries is in legally persuative with out being legal document .

living will is one form of advance directive leaving instructions for treatment .

• Most probably it is more accurate for the ptients who are at their end of their lifes .( elderly population ) .
• as a health care provider we hve to explain about the condition of the ptient to the family members .
• for example : if we tke the elderly patient who are suffering with tge lst stage of their cancer ,so the patient have the severe pain ,and he may bed ridden and he doesnt have the ability to take self care activities ,and very much immuno compromised .
• so herec,we have to explain detail health status of patient to his guardian ,and also explain about legal directive of health care .
• if tge patient is not have capacity to make the decision of his health condition ,at that time we can go for next who take the decision about health of he elderly population .
• we have to explain about palliative care for the patient ( sympomatic treatment) .
• we have to ask for the any wishes of the patient at their end of life .
• we suppose to fulfill the wishes of elderly who are at end of their life
• andalso we need to explain about organ donation of elderly who are at end of their lifes .
• if the patient is in worsen with pain ,we have to explain about mercy killing of the patient ( mercy killing done for the patient who are having unbearable condition and at the end of their lives .)
