Heart failure can determined through following physical examination -
LEFT SIDE HEART FAILURE CAN DETERMINE THROUGH FOLLOWING SIGN AND SYMPTOMS
Tachypnea, cough, crackles, wheezes, blood-tinged sputum,Confusion,Orthopnea,Exertional dyspnea and paroxysmal nocturnal dyspnea,Fatigue,Tachycardia,Restlessness.
RIGHT SIDE HEART FAILURE SIGN AND SYMPTOMS
Distended jugular veins, Dependent edema, Increased peripheral venous pressure, Anorexia, Complaints of GI distress,Ascites, Hepatomegaly and splenomegaly.
BIVENTRICULAR HEART FAILURE
In this both sides of the heart are affected. Symptoms can be same as left and right side heart failure , such as shortness of breath and a build-up of fluid.
2) Nurse should assess the Pulse Rate variation, BP variation, Respiration rate, Peripheral edema, Jugular venous pressure, Presence of extra heart sounds and murmurs.
3) RN can evaluate throgh the prescribed medication will decrease the above mentioned symptoms. After administering the medication to the patient the RN see the expected outcome.
4)The following health education can improve the patient condition.
