It's a four step model that includes the assessment of metabolic process and lifestyle, identification of nutritional goals, intervention to achieve this goal and evaluation of clinical outcomes.
This tool helps in obtaining and using these information to achieve goals and appropriate intervention to help the clients.
The components of this nutritional tool is:
1) Clinical data: Height, weight, required weight and energy requirements.
2) Nutritional assessment: Current nutritional status, choices of diet, intake of alcohol, intake of vitamin and nutrients, any eating disorders, energy intake.
3)Weight history: obtain weight history, weight changes,and weight goals.
4)Physical activity: Type of activity,energy expenditure,limitations for exercise, willingness to be active.
5)Monitoring: Assess for target blood glucose ranges, monitoring for other problem and assess client benefits from monitoring.
6)Psychosocial/ economic: Assess the socio-economic status, educational background and finances. Assess ethnics and cultural beliefs. Assess for stress level.
7) Knowledge and skill level: Assess for understanding and education level.
8) Readiness to change and expectations: assessment for willingness to accept change.
This is a basic and a very flexible tool to collect vital data to help the client to improve their awareness of health , improvement in lifestyle to promote better living.
