One Community problem - Adolescent pregnancy
Nursing process which applies for entire community ( note it can
be written in tabular form too by making same sections in
boxes)
1) Assessment
a) Subjective data - Community people explain issue of premature
death of infants in their community , hospitalization of child just
after birth, poor health of mother after pregnancy
b) Objective data - preeclampsia , premature death of infant ,
anemia in adolescent mothers , low birth weight baby , contracting
sexually transmitted disease
2) Nursing diagnosis - Infant mortality related to adolescent
pregnancy as evidenced by preeclampsia , premature death , lbw
3) Goals
- Risk of anemia will be reduced
- Premature death of infants will decrease significantly
- Risk of STD will decrease
- Community health will improve as mothers and child of family
will stay healthy
- Reduce risk of maternal school dropouts
- Increase in proper development and growth of child will
occur
4) Planning
- Assess adolescent pregnant mother and through 1 year postpartum
participants
- Identify the group at risk
- Explain the risk associated with adolescent pregnancy
- Educate pregnant mothers and through 1 year postpartum
mother
- Explain family planning , life skills and maternal role
- Monitoring of pregnant mothers and postpartum mother of under 1
year of it and infant
- Maintaining of health records of young couples
- Managing health records of community and maternal health
records
- Educate the society about adolescent pregnancy
- Provide social support by educating society
5) Nursing Intervention -
- Assess adolescent pregnant mothers , through 1 year postpartum
mothers and young couples who are at risk of getting pregnant
- Identify the group of teenages who are at risk of pregnancy and
data of their family are obtained
- Explained the risk associated with adolescent pregnancy to
family and community as it has risk of anemia in mother , infant
mortality rate, early hospitalization of infant , preeclampsia (
medical disorder in pregnant women) , premature death , low birth
weight baby.
- Eduacte pregnant mother and postpartum mother through 1 year
about importance of regular check up , medication such as
multivitamins , folic acid , iron and calcium
- Explain medical facilities available for pregnant mothers and
postpartum mother
- Monitoring of pregnant mother and through 1 year postpartum
mother by home visiting , recording health data
- Involving mothers in questionnaire , interviews to obtain
effective knowledge about adolescent pregnancy and health of child
and mother
- Explain family planning by use of contraceptive methods
available at medical health centres at free of cost , life skills
and maternal role such as importance of breastfeeding in community
, immunisation of child and how it can be done
- Managing health records of young couples , pregnant adolescent
mothers and through 1 year postpartum mothers to eliminate risk of
low birth weight baby , infant hospitalization , preeclampsia risk
can also be identified and reduced
- Educate society about girls education importance , financial
independence of women and how if one women get education it can
educate the whole family
- Provide social support to mothers by educating their family ,
society about improtance of maternal health , care of antenatal and
postnatal mothers
- Explain importance of education in everyone's life and promote
school joining of adolescent mothers to reduce risk of school
dropouts.
- Record all the health data and effectiveness of health
education and timely visiting patients home
6) Evaluation -
- Risk of anemia reduced in adolescent mothers
- Reduced cases of preeclampsia
- Reduced infant hospitalization and infant mortality
- Increase maternal health
- Reduce in school dropping of mothers
- Reduced number of adolescent pregnancy
