A.at first vist we should take the history of his disease condition..how long he has been effected with diabete?.is he taking any medication or insulin? Which diet habit he is following?is he follow the doctors instruction on diabetes?is he doing regular follow up vist?is he checking his blood sugar level every day.if we get answer for thses question we can understand that why he has this problem.so in first assessment collect information regarding his disease condition as mentioned above.

B.check that how much he can do his daily living activities.if he is bind help him on daily needs.advise him to remove all furnitures from his path in home which cause injuries .if he is blind we need to remove such things to avoid falling.tell him to follow importance of followi g doctors instruction on diabetes otherwise it may lead to complete vission loss as it destroys blood vessels in retins.if he is blind take him to the hospita to check the reson for his bindness and provide healthcare facilities.
