In: Nursing
It has to be about an organizational problem in a healthcare facility
Performance Improvement Initiative
Propose an initiative that will address this problem within the department of your chosen healthcare organization. What specific relevant quality
standard will this quality initiative address?
Discuss the data determinants of success, as related to this initiative. In other words, what type of data will be indicative of a quality outcome?
Implementation of the Plan in the
Organization
A. What interdepartmental communication channels
will be used for plan implementation?
B. What manner of data interpretation will be used
to communicate the findings within the organization?
C. If this initiative was implemented, what do you
believe would be the hypothetical effect(s) on patient care
outcomes? How will health information systems support those
improvements in patient care? D. What do you think the hypothetical
effect of the quality or performance initiative
will be on the culture of safety within the organization
Guidelines for Submission: Submit assignment as a
Word document with double spacing, 12-point Times New Roman font,
and one-inch margins.
An effective and sustainable performance improvement initiative should be formed and implemented in the context of an ongoing performance program. For such a program, the initial goals should be to prioritise performance improvement efforts. This helps the organisation to achieve early successes and also to build momentum for future performance improvement efforts.A framework "three system approach" for performance improvement is helpful with
1.an analytics system
-improving measurement and analytics
2.an adoption system
-creating permanent crossfunctional workgroup teams focussed on identifying, deploying and monitoring the effectiveness of quality improvements, and
3.a best practice system
-deploying a data-driven approach to implementing evidence-based best practices
Implementation of the performance improvement program
1.Integrating performance improvement into strategic objectives
The integration of performance improvement to the organisations strategic objectives is important since healthcare is a complex and adaptive system where interactions and relationships of different components simultaneosly affect and are shaped by the system. For instance, strategic objectives such as becoming an accountable care organisation(ACO),focussing on population health management, or developing a cardiovascular center of excellence, all require performance improvement in order to be successful. This integration also helps avoiding wasting time, effort and money on programs that may yeild little overall benefit.
2.Use of Analytics to interpret data and identify areas of opportunity
an analytics system integrates the organisations different data sources such as clinical claims, financial, operational etc.and facilitates quick and easy data sharing as well as addition of new data sources. This allows interdisciplinaryteams to analyse data and discover patterns that lead to insights..To establish identification of specific areas of opportunity among strategic areas of focus, appropriate analytics is necessary.
The 'analytics adoption model'
enable healthcare healthcare organisations to fully understand and leverage the capabilities of analytics leading to quality of care while lowering costs and enhancing clinician and patient satisfaction. As an organisation moves up the hierarchy of the analytics adoption model, data is used as an advantage and strength, helping the organisation to compete more effectively.
there are eight levels in this model-
level 8-Personalised medicine and prescriptive analytics-contracting for and managing health-analytic motive expands to wellness management, physical and behavioral functional health and mass customisation of care
level7-clinical risk intervetion and predictive analytics-taking more financial risk and managing it proactively
level6-population health management and suggestive analytics-taking financial risk and preparing your culture for the next levels of analytics
level5-waste and care variability reduction-measuring and managing evidence based care-clinical effectiveness and accountable care-analytic motive is focussed on measuring adherence to clinical best practices, minimising waste, and reducing variability
level4-automated external reporting-production of reports required for regulatory and accreditation requirements
level3-automated internal reporting-analytic motive is focussed on efficient ,consistent production of reports supporting basic management and operation of the healthcare organisation
level2-standardised vocabulary and patient registries-relating and organising the core data-master vocabulary and reference data identified and standardised across disparate course system content in the data warehouse
level1-enterprise data warehouse-foundation of data and technology-data are co-located in a single data warehouse
level0-fragmented point solutions-inefficient and inconsistent versions of the truth-vendor-based and internally developed applications to address specific analytic needs
