• Any physiological changes like alteration in eating pattern
• Neurological issues like anger ,anxiety ,lack of interest ,abnormal activities ,lack of communication or irrelevant communication
• Social changes like avoiding interactions with others ,self isolation ,not engaging in conversation
• Mental status examination to rule out basic cause

2.The specific concern here is to identify the reason behind hallucinations .Any potential risk for harming self or others.
