The patient is a 17 year old female who arrives to the emergency department in active labor with a questionable past of obstetrical history. she indicates that she has had no prenatal care, does not remember her last menstruation period and is evasive if this is her first pregnancy. Blood work indicates she is Rh negative.

A)

As her nurse I am afraid that the pregnancy affects her as well as the health of the baby. This include :

Risk for baby :

  • Low birth weight
  • Prematurity
  • Infant mortality
  • Intra uterine growth restriction
  • Haemolytic disease of newborn(due to Rh incompatibility)

Risk for mother :

  • Preeclampsia and it's complication
  • Anemia
  • Combination during child birth due to cephalo pelvic disproportion.
  • Isolation and depression
  • Chance for cesarean section.
  • Mental and financial difficulty.

B) Teachings required :

  • what changes she can expect in her body
  • Chronic conditions like hypertension that can develop
  • Importance of nutrients and healthy diet
  • Drink water more often.

Steps to take care of baby :

  • Exclusive breastfeeding
  • Looking out for complications like not feeding properly, abnormal cry, apnea etc
  • Proper energy and protein requirements.

C) Expected outcomes :

  • Anti rh antibody administered before giving birth avoids hemolytic disease.
  • Client will take adequate amounts of nutrients for the well being of the baby and herself.
  • No danger signs in newborn and exclusive breastfeeding starts within 3 days
  • Baby gains normal weight within 3 months.
