Restraints are the devices that use to Prevent patient fall and restrict the movements of patients. There are three types of restraints. Physical , chemical and environmental restraints.

Physical restraints which limits patient's movements .chemical restraints are the the form of psychoactive medications .environmental restraints also controls the patient's mobility.

It is very essential to check the Appropriateness of the type of restraints which the patient needs.

# Close monitoring.

Patients with restraints should be monitored 2 hourly. Also the the response of the patients should be checked and evaluated.

# monitoring the restrained site.

It is necessary to check the site of restrain applied site. there will be a chance to get redness, swelling and injury to the site because of the escaping tendency of the patient.

# Restrain consent.

Taking concent is very much important in restrain application. The health professional should take the concent from the first hand relatives before restrain the patient. And the it is usually a 24 hour consent.

In case of chemical restraints the nurse should assess the vital parameters of the patient atleast 2 hourly. And the reaction of the patient also.

Every day a thorough checking should be needed to assess the need of restraints.
