Below recommendations are to be included in a house wide medication safety plan:

1. Validation and proper checking: It is extremely important to check and validate the medications properly and verify the dosage and requirements of different patients before administration of medication to them. This would help in reduction of medication errors.

2. Training: It is also important to provide proper training and courses to the medical staff before letting them work on administering dosages and medication to patients. This would also help in effectiveness of medical programs and reduction of medication errors.

3. Effective recruitment strategies: Recruitment strategies have to be such that employees with adequate skillsets and experience for the job must be recruited so that human errors are minimized and medication is proper.

4. Policies and regulations: The organization must issue policies and procedures with respect to medication formulations and proper dosages so that the chances of errors are reduced to minimum.
