How do you explain the connection between electronic health record (EHR) adoption by the medical physician and personal health record (PHR) use by the patient? Does EHR use encourage patients to participate in their own healthcare? Or are proactive patients just more likely to patronize cutting-edge practices to begin with?

EHR means Electronic health record which is the digital recording system of patient details which includes the recording of Basic information about the patient ,current treatment details like in on going medicines and patient own medicines,Notes by clinician or physician, Details of patient diagnosis and its treatment and records of visits and screenings, monitor patients, and improve health care quality.

Personal health record conatin all the information included in EHRS like diagnosis,treatment medication,family history,immunization details etc but the only difference is that PHR is managed and maintained by a patient or persone himself

The major thing that is PHR is a confidential secure detail of patient maintained in his private home and confidential environment and also the self monitoring of patient own blood pressure and blood sugar also recorded by patient to know about his health statistics

The connection between PHR and EHR are

  • The PHR are made by the patient on the basics of EHR details and information
  • PHRs can include information from a variety of sources including clinicians, home monitoring devices, and patients themselves.
  • EHR are needed to prepare PHR for feasibility of recording details and to avoid medical error

EHR does not encourage the patient to participate in their own healthcare because EHR is a digital recording system It is usually maintained in Health care setting and Health care workers

EHR is not accessible by patient it is managed and maintained by health care worker and Hospital management

PHR is the only source for encourage patients to participate in their own healthcare because it gives the overall idea about health status of patient and also the patient have self study about own health status and shows progress of health

EXCERCISE 7.3

There is no specific type of persone should maintain PHR but every persone have responsibility to maintain Personal health record because it is very necessary to track and assess your health and record and track your progress toward your health goals, such as lowering your cholesterol level,controlling blood sugar etc

And the persones with acute medical condition,life style disase like diabetics,cholestrol,hypertension are more benefit from creating a personal health record.They are high risk people they have more responsibility than normal persone.
