madam Augustina Nyarko, a 45 year old Christian and
market woman weighing 82kg is on admission at the female orthopedic
ward and on bed number (3) with severe pain in the left thigh as a
result of left femur, as diagnoses by Dr Kwateng on 15th June,
2020. She is due for open reduction and internal function (ORIF) at
the theatre on 17th June, 2020. Her folder number is UGM/F134/2020.
She was knocked by a speeding vehicle on her way from the market to
her house. She has no known allergies. Her baseline vital signs
read as follows;
Temperature-38.8c, blood pressure-139.80mmhg, pulse-85bpm and
respiration-24cpm.
The physician ordered the following treatments and management;
diagnostic investigation; Blood for full blood count, fasting blood
sugar, liver function test, urine for urinalysis, x-ray of the left
femur, CT scan of the head, medications prescribed include IV
paracetamol 1g 6hourly x 24hours, IM Tramadol 50 milliliters start,
IV Normal saline 2 liters x 24 hours , IV DNS 2 liters x
24hours.
You are the the student nurse in the female Orthopedic ward working
for 24hour work.
From the above scenario, demonstrate you care within the 24hour
care of madam Augustina month ward using the nursing process as a
guide to complete your nursing care plan for madam Augustina.
Nursing diagnosis include :
1, acute pain related to fracture femur as manifested by pain scale reading and facial expression.
Goal of the interventions include reduce the pain level of patient.
Interventions include administration of analgesics, provide comfort devices, provide a proper position to the patient, assess the pain level of patient, avoid unnecessary movements and apply splint to avoid movemenms, use divertional therapies such as music therapy etc. It will help to reduce the pain of the patient.
2, fear and anxiety related to surgery
provide psychological support to the patient because of patient had fear about the surgery. Explain every procedure that we done prior to it done. An informed consent should be taken
3, impaired physical mobility and activity intolerence
assist patient in his daily living activity. We need to assist patient in basic procedures such as bathing feeding and change the position of patient periodically. Adminsiter medications as prescribed by the doctor.
