Nursing process:- Nursing process is a defined as a systematic, continuous and dynamic method of providing care to the clients.

It has 5 steps which are as follows:-

1 Assessment,

2, Nursing diagnosis,

3. Planning,

4. Implementation,

5. Evaluation.

1. Assessment:- Assessment is the 1st step of nursing process in which complete history of client is taken by the nurse. Like physical, physiological, spiritual and sociological history. This step is necessary to gather all informations and used to identify current and future patient care needs.

Nursing diagnosis:- Nursing diagnosis is the 2nd step of nursing process in which nurse evaluates data gathered in 1st step and makes the diagnosis for the needs of the client based on the informations gathered in assessment phase.

Planning:- Planning is the 3rd step of the nursing process . It involves decision making and problem solving.

Planning is the process of formulating client goals and designing the nursing interventions required to prevent , reduce or eliminate the client's health problems.

Implementation:- Implementation is the 4th step of the nursing process in which nurse provides treatment to the client on the basis of planning to improve client's health or to fulfill client's need

Evaluation:- Evaluation is the 5th and last step of the nursing process in which nurse evaluates the outcomes of the intervention in which nurse compares the client's behavioural responses with predetermined client goals and outcome criteria.

Nursing process is used in the clinical setting is cyclicaly and repeatedly during patient care until desired outcomes does not fulfill.

The sequence must be followed from start to finish to ensure that the needs of the client are fulfilled.
