Ali, 45 year old, accompanied by his brother, is admitted to Royal Hospital medical ward with complaints of severe abdominal pain, vomiting & diarrhea that started last night. He verbalized, “This is the first time I have this kind of pain”. I felt this pain all over my abdomen. I vomited 3 times already and had a loose bowel movement 2 times.
Vital signs: T: 37.5°C, PR: 112 beats/minute, RR: 25 breaths/minute: BP: 150/100 mmHg. Pain scale: 8/10.
Ali was diagnosed with Diabetes Mellitus type 1, 5 years ago. Currently, he is having insulin injection regularly and his blood sugar is well controlled.
During history taking, you observed the following: flushed & grimaced face, diaphoresis, pallor, & dry tongue & mucous membrane. He verbalized, “What is happening to me? Why I have abdominal pain? Do you think the cause was the food that we ordered from a restaurant? The food was still hot when I ate it. Good thing only me, ate the food”.
Blood tests and stool exam were done. All results were normal. The physician diagnosed Ali with Gastroenteritis. Antibiotics and IV fluids were administered to him.
Ali is working in a private company, and he has 4 young children. His wife, who is a known hypertensive, is at home taking care of their children.
Ali verbalized “I want to be discharge now. I can continue taking my medications at home. I need to go home because my wife needs me to take care of our children”. However, the physician explained to him the need to be closely monitored until he become stable.
