A 55 year old female client is scheduled for an exploratory laparotomy. The client has been experiencing abdominal symptoms for several months. A baseline assessment of all symptoms was conducted. Answer the following and explain in details to the client following: .

1. The importance of coughing, deep breathing and leg exercises

The client has also signed an informed consent for the surgery. The nurse has completed the client’s physical preparation for the surgery. The client is now being transported to the operating theatre with her records and the completed preoperative checklist.

  1. What are the nurse’s responsibilities in regard to obtaining the client’s consent for the surgery?
  2. What measures can the nurse take to allay the client’s anxiety concerning the surgery?

Exploratory laparotomy is a invasive diagnostic procedure by surgical opening of abdomen to identify injury of internal organs or diseases.This procedure is done when bleeding occcurs or patient showing abdominal symptoms

1. Importance of coughing,deep breathing and leg excercises

  • First of all this is a upper body surgical procedure that is close to heart and lungs so we needs to detect the lung efficiency and heart efficiency of patient and also to avoid complications associated with surgery for that we have to provide excercise that improves cardiac efficiency and lung efficiency
  • Breathing excercise will clear the lungs,deposited sputum in the lungs that will avoid the risk of occurance of pneumonia and other infections after surgery
  • And also deep breathing excercise will improve the efficiency of respiratory muscle it is very important because during surgery the patient has to breath with support
  • coughing excercise clear lungs
  • Before surgery maintaince of blood pressure is very important Breathing excercises help us to maintain normal blood pressure and reduced blood pressure in hypertensive patients
  • Breathing excercise will provide relief from anxiety
  • Leg excercise will improve the blood flow so it much helps during sugery to prevent complications

2. Nurses responsibility in regard to obtaining the client’s consent for the surgery

  • obtain past medical history of patient
  • obtain surgical history of patient
  • obtain anesthetic history of patient were the patient was undergone anesthesia before
  • If patient had a history of Anesthesia before obtain any related complications associated with anesthesia
  • obtain present and past medicine record of patient
  • obtain cardiac and respiratory status of patient
  • Nurse should explain about surgery and its procedure ,importance and risk of complication well
  • Nurse should verify witnessing that the patient or the family representative has signed the consent document in their presence and that the patient
  • Record pre medications of surgery

3.Measures to  allay the client’s anxiety concerning the surgery

  • Educate the client about the need and impotance of sugery going to be done
  • Explain the surgical procedure according to the age and knowledge level of patient do not tell about things that increase anxiety
  • Reassure the patient give psychological support
  • Explain about after care and rehabilation after sugery it will reduce the confusions of patient
  • Solve the confusions of patient regarding sugery
  • Enhance the patient to do deep breathing excercises
  • Tell about the experience and skill of doctors going to done surgery and quality of care going ro be provided
  • Provide diversional therapies like hearing music and watching TV etc
  • Educate about available facilities in hospital
  • Eduacte the family members about psychological support and care of patient before surgery
  • Provide anti anxiety drugs
