the barriers are-
these above points are considered as barriers to patient safety because they hinders the quality care & effcetive communication among the institution & its members.the error in reporting or any other disclosure of patient information can lead to problem for the safety which physician gave commitment to provide quality safety care.
for example- there is system of reporting errors in medical treatment to doctors & pateint but cause of fear of mistakes in the report person would blame others or try to hide the error in reports which is unethical as well as risk to saftey of the patient.
another example is poor communication & culture in the institution in which nureses or staff members do not have cooperation & coordination among each other which can create an environement of conflict & risk to pateint safety.
yes these barriers can be overcome by adopting following-
