The 5 steps of the nursing process are abbreviated to ADPIE which expands to Assessment, Diagnosis, Planning, Implementation, and Evaluation. These steps ensure that the patient receives timely and appropriate medical care.

Assessment: The registered nurse is expected to collect the patient history and present complaints including subjective and objective evidence. The data must be recorded and documented following the collection.

Diagnosis: On the basis of the data collected and the assessment, a diagnosis of the potential or actual problem must be made that the patient might have.

Planning: After the diagnosis is made the treatment approach and goals should be planned. These include immediate short term goals and patient-specific long term goals to manage the disease state.

Implementation: Once the plans are made and finalized, they must be implemented in a timely and efficient manner to achieve the treatment goals.

Evaluation: The most crucial step of the nursing process is the evaluation of the treatment goals that were planned and implemented. The nurse must evaluate the outcomes of the goals and in case of any shortcomings, approach should be made to rectify or improve it.
