Although Canada has universal health care, there are people that are not covered in the jurisdiction that they are in. How will you do this billing (which must be paid for immediately) in a way that does not upset the patient? How will you explain why your province will not cover their care without a valid card? For billing patients who are not covered under the health care in a way that they do not get upset we will have to handle the situation tactfully. The two tasks here will be not upseting the patients while billing them and abiding by the rules of the policy coverage. The rules of the health care system will have to be made clear to the patients and the importance of having a valid card as a proof of being covered in the policy. Just being a resident of Canada is not enough to be covered under the policy and a provincial health card is essential to be covered in the no payment policy. This clause will be put up to the paitent in a polite and friendly manner so he doesnt feels exploited.

A patient is a person who has already been through a tough phase recovering from the illness he was into. In such a situation he will not be mentally and emotionally prepared for any cisrcumstances that he isn't prepared for. So paying the bill because of not being covered by the health care policy or not having the card could be a unlikely situation to put up before a patient. But handling it with patience and sensibly will calm the situation and the patient will agree to pay the bill being careful about being covered in the policy in future by fulfilling its norms.
