As we know that there are different ways to record the patients and population health records that is electronic and paper records. By using electronic health records we can record the patients all health related insurance in digital form that is will record it on computer and these records could be shared between different health care settings whenever needed.Records are shared through network-connected, enterprise-wide information systems or other information networks and exchanges.
Electronic health care records include a number of data records which includes:-Demographics,medical history, medication and allergies, immunization status, laboratory test results, radiology images, vital signs, personal statistics like age and weight, and billing information.
Before couple of years, we know that electronic health care records was considered as key to quality care. The electronic health care provides on time results when the patient needs. Nowadays the electronic health records are used to improve the quality of health outcomes and their management.
Combining multiple types of clinical data from the system's health records has helped clinicians identify and stratify chronically ill patients. EHR can improve quality care by using the data and analytics to prevent hospitalizations among high-risk patients.EHR systems are designed to store data accurately and to capture the state of a patient across time. It eliminates the need to track down a patient's previous paper medical records and assists in ensuring data is accurate and legible. It can reduce risk of data replication as there is only one modifiable file, which means the file is more likely up to date, and decreases risk of lost paperwork. Due to the digital information being searchable and in a single file, EMRs (electronic medical records) are more effective when extracting medical data for the examination of possible trends and long term changes in a patient. Population-based studies of medical records may also be facilitated by the widespread adoption of EHRs and EMRs.
The principles underlying the Electronic health care system are as:-
1). The first principle is "Be objective" we have only to chart out only objective facts and avoid personal ideas in EHC.
2). Another important principle is be precise. While storing or analysing data we have to use the precise language and avoid any sort of vague language.The record should always reflect facts, and it should be written using only factual statements. Examples of generalizations and vague words include patient doing well, appears to be, status quo, as usual.
3). Make all the entries of the patient in a detailed way or thoroughly. We have to record and retrieve everything in detailed way like. course of treatment, patient condition, response to care, and deviation from standard treatment (including the reason for it). Make sure the entry is complete and contains all significant information.
4). The stored data has be kept safe for any future reference of the population.
5). While preparing reports or data the EHC management staff has to make use of abbreviation that are approved and could easily be expanded by others. It is important that everyone in the practice is using the same abbreviations for the same words.
6). There should be proper software to record all the details electronically. This software should be available in all the inter-connected hospitals and networks.
