Does anyone have the answers or a guide to a case study on i-human for David Douglas? He is a 74 y/o patient, 6' 0" (183 cm), and 165.0 lb (75.0 kg). His admission diagnosis is "Day 1 Rehab for left total hip arthroplasty, post-op day 5"

The first phase of rehablitation of client undergone total hip replacement procedure starts at 3-5 days after surgery for first two weeks . Mr.David Douglas is at the stage to start first stage rehablitation.

Aim of first stage rehablitation:

1. Pain control and reduce swelling

2. Protection of surgical hip and incision

3. Initiate pain-free joint mobility exercises

4. Reduce complications and initiate independent ambulation with assistive devices.

5. return to functional gait

interventions

1. pain control,  reduce sweeling and  protect surgical incision.

  • Administer mild analgesics
  • Ice application if needed
  • Lie flat on the bed or elevate head end 45 degree
  • Apply wraps to reduce swelling
  • Avoid showering until wound is healed

2. Joint mobility and ambulation, regaining gait.

  • Ecxercises are vital componants to initiate jount mobility and mucle strength.
  • recommended exercises are :
  • Start with active or passive form of range of motion exercise in all directions( medial, lateral, up and down)
  • Ankle rotation - rotating ankle inward outward
  • Buttocks contraction and quadriceps tightening/setting, gluteal sets
  • abduction excercises( leg slides)
  • Straight leg raising( while doing this exercise, the nounsurgical leg should be kept in bend with foot flat on bed, trighten gluteal and quadriceps muscles and knee,  then raise surgical leg straight about 12 inches above the bed level.
  • Walking - initiate walking using assistive devices.(Using cane)
  • The client should follow :
  • Do not cross operated leg
  • Acute bending shoul be avoided > 90 degrees
  • Avoid sitting in the low chairs . Instead sit on the high chairs with arm support as well as use elevated toilet.
  • Keep pillow in between the legs while sleeping
