We all know that expert opinions are the lowest level of evidence, however, these days during COVID-19 pandemic, we are guiding our treatment plan based on expert opinions and not based on EBP. Experts from Europe are saying not to give NSAIDS and steroids, and our experts are saying no difference between Motrin and Tylenol.
What are your thoughts about this level of evidence?
Answer
There have been a number of reports stating that non steroidal anti inflammatory drugs and corticosteroids may exacerbate symptoms in covid 19 patients. We have reviewed a lot of literatures on NSAIDs.
Health care providers are facing immense pressure under these conditions to treat these patients, especially those with other underlying conditions.
As far as covid 19 is concerned the acute phase of the infection can be managed with administrating NSAIDs.It is helpful in reducing symptoms associated with pro inflammatory response. But patient should follow the medication regimen instructed by the Doctor.
The NSAIDs should be prescribed carefully in diabetic patients, those who are Angiotensin-II type-I receptor blockers(ARBs),it could interfere with ACE2 expression and exacerbate the symptoms of Covid-19.
Summary
While prescribing NSAIDS,caution should be excercised until we have to receive collective evidence.Especillay in patients with Cancer and Diabetis melites. But it is effective in managing Acute phase of infection. While the use of NSAIDs can not be supported nor rejected in this context.
