A bed ridden patient with edema in their lower right leg develops a pressure induced skin ulcer in the area of the right heel. EXPLAIN what challenges the body has to overcome to fight the infection, how the body would possibly fight the infection and what would be the probable prognosis and why?
Pressure induced skin ulcers or bed sores or decubitus ulcers are injuries to skin and underlying tissue resulting from prolonged pressure on the skin . These are usually developed on the bony points of the body due to more pressure like heels buttocks shoulders etc.
Pressure induce skin ulcers are bit challenging to heal because it the most distal part of the body which is too far for blood supply and as we know blood carry all the protective cells to carry to the location of infection.
Its also depends on the state of bed sore if it is heel blisters then don't break them elevate the heel and place the leg on a splint relieve it from pressure further.
If ulcer has broken the skin then use the dressing and gauze to make the clean and moist environment to make favourable for healing and do give antibiotics orally in case infection is found to be more according to the symptoms.
Challenges will be mainly due to the most distal part of body and blood need to travel a long path to reach there in circulation.
They use to heal to normal if there is no history of any other metabolic disorder like diabetes.
This are near the heel is always in more danger to ischemia and infarction or gangere because of less blood supply so they need to be treated very carefully and proper care may heal if no such history is present.
Probable prognosis would be that if there is not history of diabetes then it will heal but it will take time and a proper attention and care. Dressing moist environment antibiotics and smart work can heal it in some time because it a ulcer due to pressure and can be treated if treated on time and carefully.
If associated with diabetes then it would be tough to heal them and or in case of negligence..
