During the examination of the abdomen a patient experiences right lower quadrant rebound tenderness. The nurse should do which additional assessment techniques for this patient?
-Assess the Murphy's sign
-Percuss to assess the organ's size
-Assess the Rovsing sign
-Test for a fluid wave
Answer : - 3. Assess the Rovsing sign.
1. Assess the Murphy's sign .
Murphy's sign indicates gall bladder inflammation.
Right lower quadrant rebound tenderness is not a sign of gall bladder inflammation.
In gall bladder inflammation the upper right quadrant pain is seen.
2. Percussion to assess the organ size.
percussion is done to identify the presence of enlargement or inflammation of organs like kidneys, liver etc.
3. Assess the Rovsing sign.
Rebound tenderness occurs in appenditis , pancreatitis etc.
In order to distinguish between them we must consider some other assessment techniques.
In this Rovsing sign , when the nurse palpates the left lower quadrant of abdomen the patient feels pain in the right lower quadrant of abdomen.
Right lower quadrant rebound tenderness and positive Rovsing sign indicates the presence of appendicitis.
4. Test for a fluid wave.
It is not needed to test a fluid wave during presence of tenderness.
----The right lower quadrant rebound tenderness and positive Rovsing sign indicates appendicitis.
