Peter Smith, a 73-year-old male with mild Type 2 diabetes, calls to schedule an appointment. He states that he is feeling very anxious and fatigued, and is having difficulty eating and sleeping. He arrives at the clinic with his wife and needs to check in.
During the patient interview, he states that he wakes up in the middle of the night, almost nightly. He also has many nights when he cannot even fall asleep. In addition, he has lost about 8 pounds in the last month. His symptoms started when his son died 6 months ago.

Peter Smith, is a 73 yrs. old male person is suffering from mild type 2 diabetes mellitus. Now a days he has some additional psychological symptoms.
1. The diagnosis that the licenced practisoner most likely give to this patient is - anxiety disorder with chronic insomnia.
2. As the patient is feeling anxiety and fatigue. He also loses his weight 8 pounds in last month. He can not sleep several nights for 6 months after the dath of his son.
We know that when a person can not sleep 3 or more days in a week and it happens for 3 consecutive months then it is called chronic insomnia. He is in a crisis situation and can not cope with the setuation. So, anxiety disorder and chronic insomnia may be the most likely diagnosis.
3. Mr. Shith complains that he was not being able to fall asleep. This information is a subjective data.
The subjective data is a type of data that is spontaneously given by the client or it may be the answer of the questions that the health care professional asked to client. This data may be include both about signs and the symptoms he is suffering from.
As in the above case the difficulty of fall asleep is informed by the client so, it is a subjective data.
4. A sleep study is a type of psychometric study that records the activity of body during sleep. This type of study is suggested for the client with sleep disorder and various other neurological disorder. So sleep study can be ordered for the patient.
