A 79-year old male patient requires anticoagulant therapy for a newly diagnosed condition and the provider has ordered both IV heparin and PO warfarin. The patient asks the nurse why he requires two anticoagulants. How should the nurse respond?
both Heparin and warfarin are anti-coagulant drugs. But in case of warfarin it works slowly in the body. It will take several days for warfarin to get effective. So, heparin is also given to compensate this duration.by acting as a coagulant. nurse have to explain about the role of both the medicine and difference of action inside body.
