MD ORDER: Insulin titration of 1 unit to 3 units/hr to keep patient's blood sugar <110. Nurse must check blood sugar every 2 hrs. The insulin IV solution is 80 units in 1L 0.9NS.
a. At what infusion rate would yo begin this titration of insulin (mL/hr)?
b. The infusion started at 1300 hrs. At 1445 hrs blood sugar is 150. Do you increase, decrease or do nothing to the titration?
c. At 1700 hrs the patient's blood sugar is 112. How do you adjust the titration? (increase, decrease, do nothing)
d. What is the maximum infusion rate that the nursee may set this insulin IV infusion? (show work)
1. 1000 ml contains 80 unit.
so, 1 ml contains 8/100 unit = 0.08 unit
and 30 ml contains 30 X 0.08 = 2.4 units/hr
As per the physician, we have to administer 1 to 3 units/hr.
So we can set the rate of infusion as 30 ml/hour.
2. If the blood sugar is 150 mg/dl, then the dose should be increased.
3. If the blood sugar is 112 mg/dl, then the dose should remain unchanged.
4.As per the physician the dose should be 1 to 3 units/hr. so we can not give more than 3 units/hr.
