at the time of patient admission,head-to-toe assessment in necessary. It includes all the body system to be assess and then you have to inform all the collected findings to the healthcare professionalto know about the pateint's overall condition. any unusual information or finding of any body system must be followed with more focused assessement of that particular body system.
For abdominal assessment you should be focused on following findings:
1) Abdominal destension: It indicate abnormalities associated with conditions regarding heart failure, pancreatitis and cirrhosis. it may also indicate about intestinal obstruction with visible peristalisis.
2) abdominal ausculate: it is related to bowl sound ( RLQ). It may be hyperactive or hypoactive.
3) Palpate: This assessment is done for pain and bladder (bowel) distension. Finding of pain and tenderness may be the indication of inflammatory conditions like peritonitis.
