An 89-years old male fell in his home, sustaining a right hip fracture. An open reduction with internal fixation was performed six days ago. The patient was discharged home, where his daughter now cares for him. The patient does not bear weight on the right lower extremity but can perform supervised pivot transfers with contact guard assistance in and out of bed. The physician orders the agency to provide physical therapy for gait training and exercise three times per week for five weeks. What codes are assigned
