Person centered care is providing individual care to patient and
their family in ways that are meaningful and valuable to them. It
includes including and informing the patient in the care without
effecting patient's privacy and integrity. The care should be
provided according to individual patient preference. Dimensions of
people centered care are:-
- Respect for patient values, preferences and expressed
needs- Every individual have there own values and beliefs,
patient too should be treated accordingly. Always make them a part
of decision makimaking and treat them with dignity,respect and
sensitivity towards their cultural values.
- Coordination and integration of care- There
should be a proper coordination in clinical care, support services
and front line patient care.
- Information and education- Patient should
always have full information about clinical status, progress,
prognosis and processes of care.
- Physical comfort- The level of physical
comfort patients report has a significant impact on their
experience.Three areas were reported as particularly important to
patients: Pain management, assistance with ADLs and hospital
environment.
- Emotional support and alleviation of fear and
anxiety- fear and anxiety associated with illness is very
common. The care providers must give attention towards it and try
comforting patient in best way possible. The best way to reduce the
anxiety of patient is by educating him about illness and
treatment.
- Involvement of family and friends- Family and
friends support is very important for the patient. The patients
family should be provided a place to stay along with patient and
should actively be involved in decision making. It is the duty of
care giver to support the family and to clear their doubts.
- Continuity and transition- Patients expressed
concern about their ability to care for themselves after discharge.
Meeting patients need in this area include teaching patient
properly about the on going medication and treatment.Provide
information regarding access to clinical, social, physical and
financial support on a continuing basis.
- Access to care- Patient should need
to know that they will recieve care whenever needed. Patient should
be provided with ease to taking appointment and scheduled checkup.
Clear instruction should be provided in when and how to get
refferal.
Example of patient centered care in hospital- Strict visiting
hours and visitor restrictions are a thing of the past in a
patient-centered care model. Patients are given the authority to
identify who can visit and when. Family members (as defined by the
patient and not limited to blood relations) are invited to visit
during rounding and shift changes so they can be part of the care
team, participating in discussions and care decisions. When not in
the room with the patient, they are kept informed of their loved
one’s progress through direct and timely updates. A
patient-centered care hospital’s infrastructure encourages family
collaboration through a home-like environment that not only meets
the needs of the patient, but also meets the needs of family
members. For example, maternity wards are being redesigned with
family-friendly postpartum rooms that can accommodate the mom, new
baby, and family members, who are encouraged to spend up to 24
hours a day together in the room to foster family bonding
