In Patient centered care ,patient is considered an capable individual with their own abilities to make decisions about their treatment.The patient centered care is associated with wide range of successful outcomes of treatment in health care. The concept of person centered care is different from a traditional treatment model where patient viewed as a passive receiver of medical interventions.In traditional model health care professionals focused on the needs of patient instead of their resources.Patient centered care model allow for inclusion of patient and their relatives in making decision about medical plan and treatments.It involves partnership between health care professionals and patient ,patient relatives from starting point of care that is from collecting medical history.
