Sandra Payne is a 66-year-old female who was admitted to surgical
unit with a history of hyperthyroidism for thyroidectomy. Despite
Mrs. Payne increase in appetite, she has lost 15 pounds within last
month. She complains of intolerance to heat and sweats profusely.
She also complains nervousness and irritability. Her vital signs
are T37.6 P 108 R 24. Recently, she has refused visitor claiming
that she is ashamed of how she looked and ask you the nurse why she
have to have surgery?
Two actual health problem:
Thyroid storm and heart disease
Thyroid storm is a life-threatening condition that increases the
symptoms of hyperthyroidism. hyperthyroidism the thyroid hormone
causes more force and speed that involves the heart that causes
symptoms like tachycardia, abnormal heart rhythm, and increases the
risk of congestive heart failure and stroke.
One potential health problem:
mania. patient irritability, anxiety, nervousness, and low
self-images, depression develop into mania.
nursing intervention:
-check the patient's blood pressure and pulse rate
continuously.
-monitor the patient cardiac condition including ECG, rate, and
rhythm.
-check the patient's temperature every 4 hours and provide calm and
cool surroundings.
-check the patient weight daily.
-provide a high-calorie diet.
-continue the thyroid medications and cardiac medication as per the
doctor's orders.
-provide adequate rest and administer sedatives if indicated.
- provide psychosocial support to the patient.
-explain the patient about the need of thyroidectomy that help them
to treat hyperthyroidsm.
-improve patient knoeldge about thyroid gland, disease process,
treatment option etc.
-enroll the patient for counselling and psychiatist opinon.
