A postoperative client reports incisional pain. The client has two
prescriptions for PRN analgesia that accompanied the client from
the postanesthesia unit. Before selecting which administer, which
action should the nurse implement
Select one
a. Compare the clients pain scale rating with the prescribed dosing
b.determine which prescription will have the quickest onset of action
c.ask the client to choose which medication is needed for the pain
d. document the client report of pain in the medical record
The correct option is
a) compare the pain scale rating with the prescribed dose.
Why because the patient has ordered for two kind of analgesic prescription from the doctor's side. Pain scale rating is usually done by the subjective data. So by comparing the pain scale assessment , a nurse can determine which prescription is suitable and effective for the client at that time.
b) we can't select the option as the suitable action for the scenario. Because, after the pain assessment only a nurse can determine which prescription will have good effect for the client.
C) why this option is also incorrect means, medicine is always a choice of doctor as per the condition of patient. We shouldn't ask for the client ' wish for having or selecting a medication. The client will be unaware or lacking knowledge about the therapeutic effect of medicine and it's courses. So this also not a right option.
D) documentation is always done last. So this option also not correct.
Hence , the first option is the wise and right action that would taken by a nurse in this kind of scenario.
