When assessing a client for early septic shock, the nurse should assess the client for which 4 findings? What is septic shock?
4 findings :-
1. Raised temperature that is fever
2. Tachycardia (increased pulse rate )
3 Hypotension decreased blood pressure leads to peripheral vasoconstriction (leading to cold extremities)
Can lead to drowsiness
4.tachypnoea(increased respiratory rate)
5.decreased urine output
Septic shock occurs when the body is effected by microorganisms and these organisms releases toxic substance which leads to changes in the body, leading to multiple organ failure.
