Ray Chan, age 24, is involved in a multiple car accident. Mr. C was wearing a seatbelt; air bag was activated. Mr. C was airlifted to the nearest trauma 1 hospital. Injuries included 3 broken ribs, broken sternum, broken left femur, contusions, open scalp wound of 4 cm. N/G tube was inserted. No previous medical history. Medication order: Tube feedings: administer ¼ strength Isocal at 60 mL/hour, total volume 240 mL.

The nurse mixes 60 mL of water with 180 mL of Isocal. What error could happen?

total fluid intake=240ml in 4hrs
total 24hrs = 240×6=1440ml fluid
only administer 60ml isocal with 180 ml water.Nurse mistaken the concentration.It will harm the patient.
