Past medical history - should ask for any history of taking medicine like typical antipsychotics chlorpromazine, promazine, haloperidol, perphenazine, fluphenazine. These drugs can cause Parkinson's disease.
Risk factors - age is an risk factor, men are tend to develop more than female, having close relative with Parkinson's increase the chance, ongoing exposure to herbicides and pesticides increase the chance of development, presence of lewy bodies
Objective and subjective data =patient's name, age ,gender, symptoms are subjective data. Signs, vitals are objective data.
Medications =levodopa, carbidopa, ropenirole, entecapone.
Laboratory data =CBC, RFT, LFT, peripheral blood smear,serum magnesium, serum calcium, vitamin D
Diagnostic and surgical intervention =Since Parkinson disease is diagnosed clinically, there is no diagnostic procedure available.
Nursing intervention =recording of vitals, giving the prescribed medicines,feeding of patient.
Client education = educate the patient and caregivers regarding the nature of disease and how much the drugs are important to alleviate the symptoms. What are the danger signs, when to contact hospital should be addressed.
Outcome /evaluation =Since the Parkinson's disease is not curable, we can controll the symptoms with drugs .
