Nursing diagnosis for bipolar disorder

1. Risk for injury related to extreme hyper activity amd increased agitation and lack of control over potentially injurious movements

Interventions:

(1). Provide safe environment by removing hazardous materials from Patient surroundings

(2). Be calm and talk to the patient in a kind low pitched voice

(3). Provide period of rest inbetween activities to prevent exhaustion and to give a feeling of security to th patient.

2. Risk for violence related to self or other directed manic excitement, delusional thinking, hallucinations.

Interventions;

(1). Encourage identification of triggers

(2). Reorient patient whenever necessary

(3). Utilize relaxation techniques.

3. Disturbed thought process related to inadequate interpretation of environment.

Interventions:

(1). Assess the extent of impairment in thinking, delusion

(2). Note the patient's attention span or difficulty in concentrating simple activities.

(3). Reorient patient to reality each time he experiences hallucination.
