He has been having headaches for the last three months but due to a hectic work schedule he has not been able to go to see his medical practitioner. During his last visit two years ago, his blood pressure was slightly elevated, but he never followed up. Upon arrival to the ED a CT scan is completed and it shows a large bleed near the frontal lobe. What should Mr. Jackson's plan of care include

Clinical manifestation.

Subjective data- Headache

Objective data- Altered level of conciousness,High blood preassure,Ct shows bleed in the brain.

Possible nursing diagnosis- Hypertension ,Cardiovascular accident.

Risk factors- High blood preassure

Medical interventions - Mainly focused on blood preassure control.

In bleeding surgery is needed.

Iv fluids is needed,physical and speech therapy is needed.

Pathophysiology - Due to some conditions blood vessel in the brain ruptures ,and leads to bleeding.It will interupt blood supply to the brain tissues .This will result in necrosis of brain tissue and leads to stroke.

Nursing interventions.-

  • Blood preassure must be monitored to avoid hypertension related rebleeding.
  • Observe for complications such as hydrocephalus,cerebral vasospasm and seizures.
  • Avoid hypothermia in hemorrhagic stroke patients to prevent vasospasm and brain damage.
  • Monitor for hypoglycemia.

Patient education.- Educate the patient regarding medication compliance and regular folloup to avoid complications.
