In social transformation of american medicine,1982,Paul starr discussed his expectation for US health sector.He predicted that US health care system will change from being physican based to co operate based.He focused on the transformation of hospitals which were indepentent, freestanding,non profit into largely centralized co opreate organization which there is no room for autonomy of individual institution and they become part of large scale enterprises with centralized co operate control.

Starr mentioned five different ways by which this transformation would occur,

1.Change in the type of ownership and control from public and voluntary non-profit to for-profit companies

.2.Horizondal integration through the development of multi-institutionalsystems with a consequent shift in the focus of control from community boards to regional and national health care cooperations.

3.Diversification and co operate restructuring from single-unit-oraganizations to polycorperate and conglomerate enterprises involved in a variety of different heallth care markets.

4.Vertical integration from acute care hospitals to organizations that provide a continuum of health care,such as health maintenance organizations(HMOs)

5.Increased industry concentration of ownership and control in regional markets and the nation as a whole.

Starr recogonized that world was changing in health care delivery,and in U.S he perceived the need for hospital oraganization to be change from the autonomous institutions.He made some predictions that could occur and much of the movements did occur,some of these stopped and reversed.
