Intake and output record means the measurement ot calculation of body fluids that enters inside the body and exists outside the body. Input and output records are very important for the assesment of the patient condition. These records are mainly maintained for the assessment of health status of person, fluid imbalances in the body, for diagnostic findings, for the referral purposes, for doctor treatment, to protect from legal cases, for documentation recordings.
For example, if the patient is administered 500ml of oral fluids through nasogastric tube feeding, then patient passes 100ml of urine either directly or through urinary catheter collection. Through maintaining of input and output records it is easier to find abnomalities and to correct the imbalances.
