Goals for Patients with problems for oxygenation, fluid and electrolytes , nutrition, elimination, rest and activity, neurosensory, protection and endocrine function:
1. Oxygenation: Patients who have problems with oxygenation may suffer with hypoxia, hypoxemia , anoxia etc.
Goals:
* Maintain adequate oxygenation of vital tissues and organs.
* To relieve hypoxemia
* To reduce the work of breathing
2. Fluid and electrolytes : The goal or expected outcome of fluid and electrolyte imbalance ,
* client will demonstrate stable fluid volume as evidenced by stable vital signs , absence of signs of edema, stable weight , balanced intake and out put.
3. Nutrition
Goals: To achieve desired weight gain / loss in a reasonable period
* progressively gains weight towards a desired goal
* identifies nutritional requirements
* weight within normal range for height and weight
4. Elimination :problems are urinary or fecal incontinence or retention
* Patient reduces or has no incontinent episodes
* Patient receives assistance for toileting in a timely manner
* Patient eliminates or overcomes environmental barriers to toileting
5. Rest and activity
Patient reports improvement in sleeping pattern
patient obtains optimal amounts of sleep ( as evidenced by rested appearance)
patient verbalizes the the rested feeling
6. neurosensory:
* Client will maintain normal Level of Consciousness
* Patient/ child will not experience injury.
7. protection
* Patient will not sustain fall / or other injuries
* Care givers will implement the strategies to increase safety and to prevent falls
* client will demonstrate selective preventive measures
7. Endocrine problems may maifest as disturbed sleeping pattern, fatigue, decreased sexual function, altered metabolism etc. goals are focusing on relieving the symptoms
* Patient will verbalize understanding of causative factors and appropriate intervention.
* Patient will be able to perform techniques to promote sleep. ( based on symptoms)
* Client and partner will express satisfaction with sexual relationship. (based on symptoms)
* Patient will verbalize the appropriate selection of nutrients in the food menu by avoiding certain type of food that is not desirable for particular endocrine disorder.
