Goals for individuals with Ineffective behaviors in
oxygenation:
- Patient’s respiratory rate remains within established
limits.
- Patient’s ABG levels return to and remain within established
limits.
- Patient indicates, either verbally or through behavior, feeling
comfortable when breathing.
- Patient exhibits growing tolerance to activities
- Patient reports feeling rested each day.
- Patient performs diaphragmatic pursed-lip breathing.
- Patient demonstrates maximum lung expansion with adequate
ventilation.
- When patient carries out ADLs, breathing pattern remains
normal.
- Patient maintain maximum tissue perfusion to vital organs as
evidenced by warm and dry skin present and strong peripheral
pulses, vitals within patients normal range, balanced intake and
output, absence edema, alert level of consciousness, absence of
chest pain.

fluid and electrolytes:
- Client will verbalize the need for dietary and fluid
restricted.
- Client will demonstrate behaviors to monitor fluid status and
prevent oror limit recurrence.
- Client will demonstrate stable fluid volume as evidevidenced by
stable vital signs, balanced intake and outpoutput, stable weight,
and absence of signs of edema.
Goals for individual with ineffective
nutrition:
- Patient designs dietary modifications to meet individual long
term goals of weight control using principle of variety balance and
moderations.
- To maintain ideal body weight.
- Maintaining normal skin turgor & muscle tone.
- Preventing the patient from developing anemia.
- Patient tolerates small and frequent diets.
Goals for individual with ineffective
elimination:
- Bowel elimination as evidenced by comfort of stool passage,
stool soft and formed, passage of stools without aids.
- Patient demonstrates behaviors and techniques to prevent
urinary retention/infection.
- Patient maintains balanced intake and output with clear odor
free urine, free of bladder distention or urinary leakage.
- Patient veralizes understanding of the condition
Goals for patient with ineffective rest &
activity:
- Patient performs activities of daily living.
- Patient performs range of motion exercises.
- Patient identifies techniques to enhance activity
tolerance.
- Patient participates willingly in necessary or desired
