A client presents to the ED with complain of chest pain, SOB, loss of appetite and mood changes over the last two days. The client is taking morphine and acetamenophen for his pain. The client is allergic to aspirin, sulfa and codeine. The attending physician orders the following medications for the client: ASA 325 mg 1 tab po stat, morphine 3 mg IV x 1 and 650 mg acetamenophen po every four hours for pain > 5/10. What would be the nurse's FIRST action.
give the medications as prescribed and monitor the client
Withhold the order and clarify the medications
Ask the patient what happens when they take aspirin
Give all the medications and a dose of beneadryl for the aspirin allergy
Medication has to given as per order and monitored but first assess the patient for its safety.so this statement is second priority .
Morphine is used to treat the pain of the patient but it can suppress the respiratory center. So before administering ensure patient's oxygen saturation ,respiratory rate are within normal limit. Here the patient is having shortness of breath .Therefore administration of this drug can further aggrevate it and lead to fatal condition. So this withhold the medicine and clarify before giving the medicine.
Asking the patient about the aspirin effect is not applicable here. Moreover he has not taken this drug before
