A hospital has a large number of registered physicians, and keeps the following personal data: physician name, date of birth, work ID and specialty. Patients are admitted to the hospital by physicians. Patient’s name and address as well as date of birth are collected. Any patient who is admitted must have exactly one admitting physician. A physician may optionally admit any number of patients. Once admitted, a given patient must be treated by at least one physician. A particular physician may treat any number of patients, or he or she may not treat any patients. Whenever a patient is treated by a physician, the hospital wishes to record the details of the treatment by including the date, time, and results of the treatment.

The above system may be modelled by the following ER Diagram. The entities are the patients and physicians. Ther are a couple of relationships as well which model the transactions. Treatment and Admissions.Admissions is a one time transaction where a Physician admits a patient if the case requires admission. There could be several physicians admitting several patients. Each patient may be treated by multiple physicians as there may be differnt problems requiring different specialties etc. All of these have been modelled in the diagram below
