There are 125 mg in 5 ml. Please answer questions 19 and 20 using this information.

* Calculation of mg of Glucophage required.
1 gm = 1000 mg.
So
mg of glucophage required = 1000 mg.
* Calculation of volume(ml).
Ordered dose = 2 teaspoon.
1 teaspoon = 5 ml.
So
Volume(ml) of drug required = 2 * 5
= 10 ml.
* Calculation of number of teaspoon.
Ordered dose = 2 tablespoon.
1 tablespoon = 3 teaspoon.
So
Number of teaspoons required = 3 * 2
= 6 teaspoon.
* Calculation of weight of the patient (pounds).
Weight of the patient in kg = 71 kg.
1 kg = 2.205 pound.
So
Weight of the patient in pound = 71 * 2.205
= 156.56 kg.
* Calculation of volume(ml).
1 fluid ounce = 30 ml.
So
Volume(ml) required for administration = 30 ml.
* Dose administered in each individual divided dose.
Ordered dose = 1 gm in 4 divided doses.
Each individual dose required = 1/4
= 0.25 gm per dose.
* Calculation of volume(ml).
Volume(ml) = Desired dose/Dose in hand * Quantity.
Here
Desired dose = 20 mEq.
Dose in hand = 30 mEq.
Quantity = 15 ml.

Volume(ml) = 20/30 * 15

Patient weight in pounds = 143 pound.
1 pound = 0.454 kg.

Volume(ml) = 125/250 * 5
= 0.5 * 5

Total amount(mg) of drug present in the vial = 2000 mg (2 gm).
Amount(mg) of drug removed = 750 mg.
So
Amount(mg) of drug left in the vial = 2000 - 750
= 1250 ml.

Volume(ml) = Desired dose/Dose in hand * Quantity.

Desired dose = 12 mg.
Dose in hand = 4 mg.
Quantity = 5 ml.

Volume(ml) = 12/4 * 5
= 3 * 5
= 15 ml.

Volume(ml) = Desired dose/Dose in hand * Quantity.
Here
Desired dose = 150 mg.

Quantity = 5 ml.
As per above formula
Volume(ml) = 150/125 * 5
= 2 * 5
= 10 ml.
Calculation of volume(ml) on days two through five.
Volume(ml) = Desired dose/Dose in hand * Quantity.
Here
Desired dose = 75 mg.
Dose in hand = 125 mg.
Quantity = 5 ml.
As per above formula
Volume(ml) = 75/125 * 5
= 0.6 * 5
= 3 ml on day two through five.
