Shared decision making (SDM) is a process in which the health care professionals and patient together takes a decision regarding the disease condition. collaboratively help patients to reach evidence-informed and value-congruent medical decisions. It helps the patient to differentiate between the harm and benefits of purticular treatment aspects.The patient explains their compliants to the clinicians, together they select the diagnostic tests, treatments and careplans based on the appropriate evidence. It gives importance to the patient preferances and values. Overall it helps to provide a positive outcome. When patients participate in decision making and understand what they need to do, they are more likely to follow through. This process empowers the patients to make decisions that are right for them.During shared decision making, it's important that care or treatment options are fully explored, along with their risks and benefits.Patients who are empowered to make decisions about their health that better reflect their personal preferences often experience more favourable health outcomes. This can include being less anxious a, quicker recovery and increased compliance with treatment regimens.

In electronic medical records, all the datas are entered and stored in to the computer. Remote patient monitoring means the information is shared through telephone. In written reports, the datas are written and shared in between health care team.
