3.Imbalanced nutrition less than body requirement related to insufficient dietary intake as evidenced by anorexia.
4.Activity intolerance related to weakness and muscle ache as evidenced by inability to do activity.
5.impaired urinary elimination related to disturbance in tubular function secondary to Addison insufficiency.
6.Deficient knowledge regarding therapeutic regiment as evidenced by patient verbal response.

11. Who would benefit from Sodium Polystyrene? Possible side effects and patient teaching?
Benefit from sodium polystyrene
The medication is used to treat a high level of potassium in blood. Too much potassium in blood can sometimes cause heart rhythm problems. Sodium polystyrene sulfonate works by helping body get rid of extra potassium.
Side effects
Loss of appetite
nausea and vomiting
constipation may occur.
Diarrhea may occur less frequently.
Hypokalemia
Hypocalcemia
Hypomagnesemia
Patient teaching
1.Encourage the patient to eat dietary fiber, drink enough water, and exercise to prevent constipation.
2.Advice the patient to take a laxative.
3.Instruct the patient to report signs of an allergic reaction, like rash,hives,itching,redred, swollen, blistered, or peeling skin with or without fever; wheezing; tightness in the chest or throat; trouble breathing, swallowing, or talking; unusual hoarseness; or swelling of the mouth, face, lips, tongue, or throat.
4.Instruct the patient to avoid intake of high pottasium diet
