J P is a 45 yrs. old man, admitted in the out patient department. He is a truckdriver and he had no injury in foot.
The assessment that should be done =
i. The history has to take in detail from the patient. The history about duration of time he drives the truck. The history of his food habit, alcohol consumption, smoking, or any drug addiction has to take in detail.
ii. Some laboratory studies has to be done for this patient likes as complete blood count, blood urea and creatinine level, blood electrolyte balance, x-ray of right foot.
The most probable cause of edema for this patient is immobility of leg. The client is a truck driver so, he may has to have a long drive for one place to another. There is a high chance of less mobility of the patient's legs, mostly the right leg as, during driving the left leg is used more than the right leg. So, there is a chance of developing edema at his leg.
The nursing diagnosis will be -
1, pain related to edema at the right leg, low circulation of blood as evidenced by patient's verbalisation and numerical pain scale measurement.
Interventions =
- The location, frequency, duration, level of pain has to measure.
- the hot water bag has to place at the site of pain to reduce the pain and increase blood circulation.
- Analgesics has to be administered as per physician prescribed to reduce pain.
2. Swelling of right ankle related to less mobility of leg and less physical activities as evidenced by taking patient's occupational history.
Interventions =
- The site of swelling, severity of edema has to be assessed to collect basic information form the patient.
- The intake and output chart has to be monitored in a proper time interval to monitor the fluid balance of body.
- Osmolytics has to be administered as per physician prescribed to reduce edema.
- proper foot elevated position has to provide to the patient to reduce swelling.
